We could not find a dose-reconciliation claim on pmhscribe.com, so we show what it looks like in Psynopsis. A follow-up that says “continue lamotrigine” is reconciled against the last plan; a draft reading 20 mg when the previous note said 200 mg is flagged before you sign.
